    We used to have a ‘no red’ ink policy, because back in the day, photocopiers didn’t photocopy red ink very well. Then we had a ‘no black’ ink policy, because it made it too hard for the paper pushers to distinguish between the copy and the original. Nurses had to use ‘blue ink only,’ while the techs had to use green ink.

    This has all went away with computerized charting and better copiers, but has anyone else have examples of truth is stranger than fiction policies?
